South Eastern Community Connect (SECC) is seeking an experienced and motivated Support at Home Team Leader to lead our growing aged care services team. This is an exciting opportunity for a proactive and results-focused professional who enjoys building strong client relationships, identifying opportunities to enhance service delivery, and leading a team committed to exceptional care. The role is responsible for client intake and onboarding, care management, team leadership, quality assurance and supporting program growth.
About the Role
This is not a role for someone who simply maintains a caseload. We are looking for an energetic and solutions-focused leader who takes initiative, builds meaningful relationships with clients and families, and proactively identifies opportunities to maximise client outcomes and Support at Home package utilisation.
You will oversee client intake, assessments, onboarding, care planning and service coordination, ensuring clients receive timely access to services and ongoing support that aligns with their goals, preferences and changing needs. The role also provides day-to-day leadership and support to Care Partners while maintaining an allocated client caseload.
Key Responsibilities
Lead and support a team of Care Partners to deliver high-quality, person-centred care.
Manage client intake, assessments, onboarding and care planning from enquiry through to service commencement.
Ensure new referrals and clients are responded to promptly and efficiently, with a strong focus on timely onboarding and service commencement.
Build strong relationships with clients, families and representatives to understand their goals and changing needs.
Monitor Support at Home package utilisation and identify opportunities to enhance service engagement and outcomes.
Coordinate and review client care plans to ensure services remain responsive, appropriate and outcomes-focused.
Identify opportunities to improve client outcomes, service quality and package utilisation through regular client reviews and ongoing engagement.
Support quality improvement, compliance, accreditation and continuous improvement initiatives.
Contribute to client retention, service growth and a positive client experience.
About You
You are a motivated and proactive professional who enjoys achieving outcomes and creating positive experiences for clients. You are confident engaging with clients and families, can identify unmet needs and opportunities for additional support, and have a strong commitment to ensuring clients receive the maximum benefit from their Support at Home package.
You will bring:
Experience in aged care, community care, case management or care coordination.
Demonstrated leadership or supervisory experience.
Strong assessment, care planning and relationship management skills.
A proactive approach, strong organisational skills and the ability to manage multiple priorities in a fast-paced environment.
Experience improving client outcomes, service utilisation and customer satisfaction.
Knowledge of the Aged Care Quality Standards and contemporary aged care practices.
A commitment to person-centred care and continuous improvement.
