St Vincent's Care Services Toowoomba - provides comprehensive 24/7 nursing, dementia, and palliative care within a peaceful, community-focused environment featuring landscaped gardens, an on-site chapel, and active lifestyle programs.

The role of Regional Clinical Funding Coordinator (RCFC) is to ensure the clinical accuracy of all SVCS clinical assessments and care plans for all residents in SVCS homes.

RCFCs must work closely with the home's clinical and care team ensuring all documentation for admissions and existing resident care reviews (reclassifications) are conducted under an open, transparent, and systematic approach that appropriately balances and reflects the resident's care needs, their choices, and the quality standards governing Residential Aged Care.

Key Responsibilities of the role:

- Accurately determine resident's health issues, needs, and functional levels using a multidisciplinary approach with a consumer-centred focus.
- Liaise with various stakeholders to ensure a person-centred focus for care services and appropriate funding levels in accordance with current clinical and functional levels.
- Evaluate the quality of clinical documentation and care planning against the Aged Care Quality Standards.




- Support the timely completion of aged care assessments and care plans for new and existing residents.
- Monitor, document, and manage the gradual or acute increase in a resident's clinical care needs and other significant clinical events.
- Support the accurate documentation and reporting in aged care business portals, such as Medicare (Services Australia), Aged Care Portal records (My Aged Care Portal), AN-ACC bookings, Sharepoint etc.
- Work collaboratively and communicate effectively as a member of a multidisciplinary team, leading a systematic approach to evidence-based practice in aged care.
- Mentor, educate and support the training needs of clinical and personal care teams to ensure accurate and timely documentation of clinical needs and changes align with SVCS policy and industry standards of care and funding.
- Support the documentation and timely reporting of regulatory and industry benchmarks such as Barthel Assessments, Cognitive Assessments and funding performance.
- Travel occasionally (when required) to meet consumers, care teams, and business support teams, fostering a culture of open communication and professional accountability.

Things that work for us:

Essential

- Registered with AHPRA (Australian Health Practitioner Regulation Agency) as a Registered Nurse essential.
- Significant knowledge of the AN-ACC funding model and how it contributes to contemporary residential aged care environments




- Demonstrable experience performing aged care clinical assessments and care plans
- Demonstrated experience supporting an aged care business', regulation and funding processes
- Appropriate level of immunisation as a Registered Nurse (Covid-19, Influenza)
- Ability to travel intra and interstate (if required) to meet consumers on-site and support internal and external stakeholders involved in the completion of aged care assessments and care plans
- Intermediate computer skills (MS office and apps) to deliver face to face and telehealth services necessary for the timely completion of aged care assessment and AMO funding reviews.
- National Criminal History Check (valid within 6 months) and NDIS Worker Screening Check will be a requirement of this role.

- Robust written and communication skills to work effectively with internal and external stakeholders.
- Ability to maintain collaborative and effective relationships with all multidisciplinary team members
- Ability to work flexibly within the Facility and other SVCS homes (if required)
- Ability to provide clinical practice and care consistent with the mission and philosophy of SVCS
- Ability to plan and coordinate resident care effectively, maintain resident advocacy and utilise resident management systems
- Ability to participate in and support education activities and to contribute to the development of self and other team members
- Have a working knowledge and understanding of relevant regulations and legislation
- Ability to contribute towards effective risk management within the facility particularly in compliance with Work Health & Safety legislation
- Demonstrated application of contemporary infection control standards and practices
- Ability to participate in quality improvement activities
