The Institute for Urban Indigenous Health (IUIH) is seeking a Care Coordinator CCSS - MATSICHS to support Aboriginal and Torres Strait Islander communities across Caboolture, Morayfield, Strathpine, Deception Bay and Margate. You will work within a multi-disciplinary team delivering specialist community and primary health nursing services to prevent, detect and manage health problems.

Based at designated clinics and outreach sites, you will perform triage, routine screenings, wound care and home
