Dhelkaya Health in regional Victoria is seeking a Community Nursing Clinical Coordinator to provide operational and clinical leadership for the District Nursing and Community Palliative Care teams in Mount Alexander Shire.

You will coordinate triage, intake, care planning and clinical interventions while supporting staff development and quality improvement initiatives. This is a part time, ongoing role with a focus on delivering high-quality, person-centred care.
