What you'll be doing
Assess, plan,



implement and evaluate community nursing care, in the patient home or clinic, in collaboration with individual/s and the greater health care team to achieve quality patient outcomes.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:

- Registered Nurse –Division 1 with Australian Health Practitioner Regulation Agency (AHPRA); and Current NSW Drivers Licence.
- Demonstrated ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers.

Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.

This is a NSW Health Category A position which requires immunisation and screening for certain diseases.
