Bayside Health is seeking a Social Worker for the HIV Outward Program to deliver psychosocial assessment, counselling, and casework across inpatient, outpatient, and community settings. You will collaborate with Nursing, Medical and Mental Health Clinicians in a dynamic, multi-disciplinary team.

The role focuses on people living with HIV, addressing adjustment to illness, stigma, and complex care needs, with a patient-centered approach and solid engagement with community links.
