As a Care Coordinator, you will play a crucial role in supporting valued clients, particularly those in Supported Independent Living (SIL) settings with diverse disabilities and complexities. You will be responsible for overseeing their care plans and working closely with their support networks and care teams to ensure their needs are met.
Key responsibilities
- Effectively communicate with the participant's support team, including the Support Coordinator, guardians, advocates, family members, and ACRS Community Services Manager
- Conduct regular meetings with the care team to review and implement care plans effectively
- Coordinate with the Rostering and HR departments to address support worker issues or recruitment needs
- Monitor and ensure the delivery of high-quality care by reviewing participants' reports, and care plans, and addressing any issues promptly
- Ensure that all support workers fully understand and meet the participants' needs in line with ACRS Community Services' values
- Report directly to the Relevant Manager and work collaboratively with other team members
- Perform regular SIL property inspections and address any identified issues
- Maintain accurate participant and team records in ACRS Community Services' record system
- Handle the onboarding process for recent clients, including initial assessments and compliance documentation
- Engage with participants, their networks,
and other stakeholders to streamline the transition process
About you
- Qualifications in Disability Services (Minimum Certificate IV in Disability or Diploma in Community Services)
- Proven experience in the disability sector, specifically within NDIS services and SIL settings
- Understanding of NDIS guidelines and support structures
- Knowledge of relevant legislation and safety regulations
- Strong communication and team engagement abilities
- Excellent organizational skills and meticulous attention to detail
- Ability to work both independently and collaboratively
- Commitment to upholding the rights and dignity of individuals with disabilities
- Computer proficiency, especially in Microsoft Office
- Necessary certifications
