About the role

OSAN Ability is seeking an experienced and motivated Case Manager – Support at Home to join our Broadmeadows team. This is a hands-on role with a strong focus on client intake, onboarding, case management and service coordination. You will be responsible for responding to new enquiries and referrals, guiding prospective clients and families through the intake process, completing initial assessments, preparing service agreements and care plans, coordinating commencement of services and then providing ongoing case management.

Key responsibilities

- Manage new Support at Home enquiries and referrals from initial contact through to service commencement
- Respond promptly and professionally to prospective clients, family members and representatives
- Contact new referrals and conduct initial intake discussions to understand the client's needs, goals and circumstances
- Explain OSAN Ability services, pricing, fees and the Support at Home service model clearly to clients and families
- Conduct initial client assessments and identify care, safety, risk and service requirements
- Arrange and conduct initial client meetings, including home visits where required
- Prepare and coordinate service agreements, consent forms and onboarding documentation
- Develop initial care plans and service schedules
- Coordinate with rostering to identify suitable support workers and commence services promptly
- Manage a portfolio of Support at Home clients and conduct comprehensive assessments

About you

- Previous experience in aged care case management, client intake, onboarding or service coordination




- Experience with Home Care Packages, Support at Home or community aged care
- Strong understanding of My Aged Care and the Support at Home Program
- Confidence managing new client enquiries and conducting intake conversations
- Strong assessment and care-planning skills
- Experience coordinating support workers, allied health professionals and external providers
- Excellent communication and relationship-building skills
- Solid follow-up skills and ability to manage multiple referrals simultaneously
- Excellent organisational and time-management skills
- High-level computer skills, including Microsoft Office

Requirements

- Current Australian Driver Licence
- National Police Check
- NDIS Worker Screening Check, or willingness to obtain
- Current First Aid and CPR, or willingness to obtain
- Valid working rights in Australia
- Ability to meet applicable aged care worker screening requirements
