The CompanyForHealth is one of Australia’s leading primary healthcare providers, operating medical centres nationwide and delivering quality, accessible care to local communities.We are seeking an experienced and patient-focused Registered or Enrolled Nurse to join our Deception Bay Medical Centre on a part time basis, specialising in Chronic Disease Management and preventative healthcare.The RoleAs our CDM Nurse, you will work closely with GPs, nurses, allied health professionals and patients to coordinate personalised care for people living with chronic health conditions.You will empower patients to take greater ownership of their health through education, goal setting, ongoing monitoring and practical support.Key ResponsibilitiesPrepare GP Management Plans and Team Care ArrangementsComplete patient histories, biometrics and chronic disease assessmentsWork with patients to establish practical SMART health goalsDevelop action plans and coach patients through sustainable health-behaviour changesCoordinate referrals to allied health professionals and specialistsConduct preventative health assessments, including:Heart health assessmentsDiabetes risk assessments45–49-year-old health assessmentsOver-75 health assessmentsAboriginal and Torres Strait Islander health assessmentsIntellectual disability, veteran and refugee health assessmentsProvide ongoing monitoring and support to eligible patientsPerform clinical services such as ECGs, spirometry, urinalysis and Ankle Brachial Index testingCollaborate with GPs and multidisciplinary teams to deliver coordinated,
