Lead Positive Change in Aged Care
STAR Community Services is seeking an experienced and passionate CHSP Program Team Leader to provide leadership and operational oversight across our Commonwealth Home Support Programme (CHSP) services.
This is an exciting prospect to lead a dedicated team delivering essential supports that help older Australians maintain their independence, safety, and connection to their communities. As the aged care sector transitions to the new Support at Home program, you will play a key role in guiding service excellence, supporting staff, and ensuring compliance with evolving industry standards.
About the Role
Reporting to the Client Planning and Service Delivery Manager, you will lead and support Coordinators responsible for a range of CHSP-funded services, including:
- Domestic Assistance
- Home Maintenance
- Home Adjustments
- Social Support (Group)
- Social Support (Individual)
- Assistance with Care and Housing (ACH) – Hoarding and Squalor
You will provide strategic and operational leadership to ensure services are delivered safely, efficiently, and in line with CHSP guidelines, STAR's values, and the Strengthened Aged Care Quality Standards.
Key Responsibilities
Leadership & Team Development
- Lead, mentor, and support a team of Coordinators and service delivery staff.
- Foster a culture of accountability, collaboration, and continuous improvement.
- Support workforce capability development through coaching, supervision, and training.
- Prepare teams and services for the implementation of the Support at Home program.
Program Management & Service Delivery
- Oversee the delivery of all CHSP service streams.
- Ensure services are person-centred, reablement-focused, and responsive to participant needs.
- Monitor service quality, participant outcomes, and program performance.
- Coordinate relationships with contractors, associated providers, and community partners.
Governance, Compliance & Quality
- Ensure compliance with CHSP funding requirements, quality standards, and reporting obligations.
- Oversee assessments, care planning, documentation, and review processes.
- Manage program budgets and contractual obligations.
- Lead continuous improvement initiatives using participant feedback and service data.
Stakeholder Engagement
- Build strong relationships with My Aged Care, health professionals, community organisations, and regulatory bodies.
- Support participants experiencing complex challenges, including social isolation, hoarding, and housing vulnerability.
- Contribute to service planning, growth opportunities, and aged care reform readiness.
About You
You are a motivated and capable leader with a strong understanding of aged care and community services.
Essential Requirements
- Tertiary qualification in Social Work, Community Services, Aged Care, Allied Health, or a related field, or significant equivalent experience.
- Demonstrated experience coordinating or managing CHSP, aged care, or community-based programs.
- Knowledge of CHSP guidelines, funding requirements, and aged care compliance frameworks.
- Experience leading teams and building high-performing workplace cultures.
- Strong stakeholder engagement and relationship management skills.
- Understanding of wellness, reablement, and person-centred practice.
- Excellent organisational, communication, and problem-solving abilities.
- Intermediate computer skills and experience with client management systems.
Highly Regarded
- Experience in home modifications and building compliance processes.
- Knowledge of mental health, housing support, occupational therapy, or hoarding and squalor interventions.
- Experience managing change within reform-driven environments.

Mandatory Requirements
Successful applicants will be required to hold or obtain:
- Current Driver Licence and access to a reliable, registered, and comprehensively insured vehicle.
- NDIS Worker Screening Check.
- NDIS Worker Orientation Module Certificate.
- Current First Aid and CPR Certification.
Current influenza and COVID-19 vaccinations are strongly encouraged.
