About The Role The Clinical Nurse Consultant (CNC2), Older Persons Mental Health (OPMH) Community services undertake specialist mental health assessment, care planning, short- and longer-term clinical management (involving clinical care and coordination, treatment, clinical review and transitions of care) and consumer advocacy, for older people (65+ and 50+ for Aboriginal and Torres Straight Islanders) with mental illness.

Providing assessment, care planning and review of patients in collaboration with the primary service providers to ensure that evidence based practice is utilised. Providing both planned and urgent consultation for complex or contentious consumers.

The Clinical Nurse Consultant (CNC2) is responsible for expert clinical organisation, coordination, leadership and consultancy services within the Older Persons Mental Health team and target populations within the Northern NSW Local Health District (NNSWLHD).
