We're looking for an experienced AHPRA Registered Nurse to join our Clinical Team as a Community Clinician in a permanent part-time capacity.
This is a highly autonomous role where you'll work directly with clients, support workers, families and allied health professionals to ensure safe, quality and person-centred care outcomes.

About the Role
As a Community Clinician, you will be responsible for overseeing the delivery of safe, high-quality clinical services to clients living in the community. You'll provide clinical leadership, complete assessments, develop care plans and work collaboratively with internal and external stakeholders to achieve the best possible outcomes for clients. Key Responsibilities
Clinical Care & Case Management
- Conduct client assessments and reviews
- Develop, implement and monitor care plans
- Coordinate and facilitate clinical interventions and support services
- Liaise with GPs, specialists, allied health providers, families and carers
- Monitor client wellbeing and progress against care goals
- Advocate for clients and promote person-centred care approaches
- Support intake and onboarding of new clients
Clinical Leadership & Education
- Provide mentoring and clinical guidance to support workers and nursing staff
- Deliver education and training sessions to internal teams
- Support competency development and clinical best practice
- Promote continuous improvement and evidence-based care
Quality, Compliance & Risk Management
- Support clinical governance, accreditation and quality improvement activities
- Assist with audits, incident management and compliance initiatives
- Maintain accurate client documentation and records
- Contribute to policy, procedure and service improvement activities

About You
We're seeking a compassionate and experienced clinician who enjoys building relationships and supporting people to achieve positive outcomes.
You'll bring
- Current unrestricted AHPRA registration as a Registered Nurse
- Minimum 3 years' experience within disability, aged care, community or healthcare settings
- Strong understanding of NDIS, community care and person-centred practice
- Experience conducting client assessments and developing care plans
- Excellent communication and stakeholder engagement skills
- Strong clinical judgement and problem-solving ability
- High attention to detail and compliance focus
- Current driver's licence and access to a reliable vehicle
Essential Requirements
- AHPRA Registered Nurse registration
- NDIS Worker Screening Check
- Working With Children Check
- Current CPR Certification
- Ability to travel within the community to meet with clients and stakeholders
