Join the Southern Adelaide Local Health Network as a part-time Clinical Nurse - Diabetes Educator, delivering community-based education and self-management support to people living with diabetes. Working within a collaborative multidisciplinary team at the GP Plus Marion and Noarlunga unit, you'll conduct clinical assessments, facilitate structured education programs, and help patients build the confidence and skills to manage their health long-term.
Roles and Responsibilities
- Providing proficient, person centred, clinical diabetes nursing care and individual case management to patients in the Out of Hospital Service
- Managing Diabetes related technology in both the inpatient and outpatient setting
- Participating in clinical teaching, overseeing learning experience, and goal setting for students, new staff and staff with less experience
- Delivering Group education sessions to patients, staff and community groups under the supervision of the RN3
Requirements
- To be Registered or eligible for registration as a Nurse and Midwife by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate
- At least 3 years, full time equivalent, post registration experience
- Competence in Diabetes Education and Management nursing practice in accordance with the relevant standards
- Experience in the leadership and direction of student nurses, enrolled nurses and less experienced registered nurses
