About the role

You will provide personalised care and support in the client’s home, helping to maintain their comfort, dignity and quality of life. Duties may include:





- Providing compassionate personal care and assistance with daily activities
- Supporting comfort and wellbeing
- Monitoring and reporting changes in the client’s condition
- Supporting family members with a calm and reassuring approach
- Following individualised care plans and clinical instructions
- Maintaining accurate documentation and communicating with the wider care team
- Providing respectful, dignified end-of-life care
