About the role
As a Disability Support Worker with YH Support, you'll work directly with NDIS participants to help them achieve their goals and build greater independence in everyday life. Your day-to-day support may include personal care and daily living activities, community participation and access, household tasks and meal preparation, life skills development, social and recreational activities, medication support where appropriately trained and authorised, Supported Independent Living (SIL), transport and community access, and supporting participants to implement their individual support plans.
Key responsibilities
- Provide high-quality, person-centred support
- Follow individual support plans, care plans and Positive Behaviour Support (PBS) Plans
- Support participants to work towards their NDIS goals
- Promote independence, choice, dignity and community inclusion
- Monitor and report changes in participant wellbeing or presentation
- Complete accurate and timely progress notes and incident reports
- Follow organisational policies and NDIS requirements
- Maintain professional boundaries and confidentiality
- Communicate effectively with participants, families, coordinators, supervisors and other professionals
- Work collaboratively as part of a multidisciplinary support team
About you
- Compassionate, patient and respectful
- Reliable and punctual
- Confident supporting people with complex needs
- Strong written communication and report writing skills
- Comfortable following support plans, policies and procedures
- Able to work independently and as part of a team
- Versatile and willing to work across a variety of shifts
- Committed to person-centred practice and the rights of people with disability
- Previous experience as a Disability Support Worker or in a similar role
- Certificate III in Individual Support (Disability) or equivalent, or willingness to obtain this qualification
Essential Requirements
- NDIS Worker Screening Check
- Working with Children Check (where applicable)
- Current National Police Check
- NDIS Worker Orientation Module completion
- Current First Aid Certificate
- Current CPR Certificate
- Australian driver's licence
- Reliable, insured vehicle, where required for the position
- Ability to work legally in Australia
