We are seeking an experienced and compassionate Neighbourhoods Intake & Triage Officers to join our Housing & Neighbourhoods team.

Working across our three sites, Kallangur, Lawnton and Redcliffe this important role is often the first point of contact for community members seeking support. Through skilled assessment, triage, service navigation and referrals, you will help ensure people receive the right support at the right time.

About the Role

The Neighbourhoods Intake & Triage Officer provides intake, screening, assessment, triage and service navigation support for individuals and families accessing Encircle's Housing & Neighbourhoods services. Working within a strengths-based, trauma-informed and person-centred framework, you will support early intervention, homelessness prevention, tenancy sustainment and improved community wellbeing outcomes.

Key responsibilities include

Intake, Assessment & Triage

- Act as the first point of contact for community members seeking support.
- Conduct intake, screening and initial needs assessments.
- Identify presenting issues, strengths, priorities and support needs.
- Assess urgency and facilitate timely responses based on identified risk and need.
- Escalate complex or high-risk matters in accordance with organisational procedures.

Service Navigation & Referrals

- Provide information, advice and service navigation support.
- Facilitate referrals and warm referrals to internal and external services.
- Support access to housing, homelessness, community and wellbeing services.
- Promote community participation, engagement opportunities and support programs.

Stakeholder Engagement & Administration

- Build effective relationships with community organisations,



service providers and government agencies.
- Maintain accurate client records, case notes and service data.
- Utilise client management systems and contribute to reporting and service monitoring activities.
- Support integrated and coordinated responses across the Housing & Neighbourhoods team.

What We're Looking For

Essential

- Qualification in Community Services, Human Services, Social Work or a related discipline, or an equivalent combination of qualifications and experience.
- Experience in intake, assessment, client support, community services, housing, homelessness or related sectors.
- Experience supporting individuals and families experiencing vulnerability, housing stress or complex needs.
- Strong intake, assessment and triage skills.
- Knowledge of housing, homelessness, community services and referral pathways.
- Excellent communication and stakeholder engagement skills.
- Strong administration, record management and digital literacy skills.
- Commitment to strengths-based, trauma-informed, culturally safe and inclusive practice.

Mandatory Requirements

- Current Queensland Driver Licence.
- Current Blue Card or ability to obtain.
- Current National Police Check or ability to obtain.
- Right to work in Australia.

Selection Criteria

1. Intake, Assessment & Service Navigation Demonstrated experience undertaking intake, assessment, screening, triage and service navigation activities with individuals and families experiencing complex needs.

2. Housing & Community Services Knowledge

Knowledge of housing, homelessness, tenancy sustainment and community support systems, including referral pathways and coordinated support responses.

3. Client Engagement & Practice Frameworks

Demonstrated ability to apply strengths-based, trauma-informed, person-centred and culturally safe practices when engaging with clients and responding to identified needs.

4. Risk Assessment & Response

Demonstrated ability to identify, assess and respond appropriately to risk, safety and wellbeing concerns, including escalation and safeguarding requirements.

5. Communication & Stakeholder Engagement

Highly developed communication and relationship-building skills with the ability to collaborate effectively with clients, colleagues, government agencies and community service providers.

6. Administration & Data Management

Experience maintaining accurate records, managing client information, preparing reports and using client management systems and digital platforms.

7. Diversity & Inclusion

Demonstrated commitment to working respectfully and effectively with priority communities, including First Nations peoples, CALD communities, LGBTQIA+ people, people with disability and other vulnerable groups.

8. Organisation & Teamwork
