Uniting Communities is seeking a Triage Assessor for its ACAS team in a fully remote role. You will perform phone-based evaluations to determine eligibility for aged care support, allocating clients to suitably credentialed assessors and guiding urgent referrals while working within established frameworks.
You will hold a health-related tertiary qualification and maintain registration or eligibility with AHPRA/AASW; experience in aged care assessment is preferred and you will use My Aged Care
