ABOUT THE ROLE
We are seeking an experienced Support Coordinator who is a motivated self starter, with a passion for achieving measurable outcomes for vulnerable people and a proven track record of providing ethical and appropriate support.
As a Support Coordinator, you'll be a crucial part of our close-knit team, working to support NDIS participants and their representatives. We believe in manageable caseloads that allow you to provide genuine, high-quality support to each participant.

Your experience in support coordination has shown you the importance of maintaining high standards while providing genuine, personalised support.
You are a natural problem solver, and are adept at responding to an ever-changing NDIS landscape.
You are reliable, self-motivated and have exceptional time management skills.
You are able to write high quality, impartial, evidence based reports.
You are able to complete home-visits in the greater Brisbane and Gold Coast areas.

KEY RESPONSIBILITIES In this role, you'll have the prospect to make a meaningful difference in people’s lives by:
- Partnering professionally with participants and their families to implement,
monitor and review NDIS plans effectively.
- Facilitating connections between participants and community resources, mainstream services, and informal supports to enhance their independence.
- Coordinating various support elements - from informal networks to funded services - ensuring participants have comprehensive support systems.
- Supporting participants to achieve their goals through ethical, participant-centred practice.
- Maintaining clear, professional documentation that reflects participants' progress while meeting compliance requirements.
- Developing effective working relationships with participants, their families, and their trusted service provider network.
- Gather and collate evidence from all formal and informal supports to produce high quality reports.
- Using your expertise to enhance our service delivery through continuous improvement.
ESSENTIAL REQUIREMENTS
- Minimum 2 years experience in NDIS Support Coordination
- Strong ethical framework and commitment to participant-centred practice
- Relevant qualifications/experience in Allied Health, Social Work, Community Services, or related field
- Excellent communication and relationship-building skills
- Ability to work autonomously while remaining connected to our team
- Current Driver's Licence and ability to travel within the Brisbane/Gold Coast region for home-visits
- NDIS Worker Screening Clearance Check and Working with Children's Check (employee check)
