Hope & Care Community Services is seeking passionate, compassionate and reliable Support Workers to join our team across Brisbane and surrounding areas. We are looking for people who are committed to providing respectful, person-centred support and helping participants maintain their independence, wellbeing and connection with their community.
Key responsibilities
- Provide person-centred support to participants in their home and community.
- Assist with daily living activities, personal care, personal hygiene and meal preparation as required.
- Support participants with household duties, shopping and community access.
- Encourage independence and support participants to maintain meaningful and positive daily routines.
- Assist participants to participate in community, recreational and social activities.
- Build respectful and supportive relationships with participants, their families and support networks.
- Follow individual care plans and provide support according to participant needs and goals.
- Complete required administrative tasks and documentation.
- Work collaboratively with other team members and communicate effectively with participants and their support networks.
- Maintain a secure, clean and respectful environment.
About you
- Current C Class Driver Licence.
- Access to a registered and roadworthy vehicle.
- Comprehensive motor vehicle insurance.
- Current NDIS Worker Screening Card.
- NDIS Worker Orientation Module Certificate.
- Good communication and interpersonal skills.
- A genuine commitment to supporting people with disability and promoting their dignity, independence and rights.
- Experience in disability support, mental health or psychosocial support, youth work, aged care, community services, personal care, or supporting people with complex support needs is highly regarded.
