We are currently welcoming Expressions of Interest for a secondment prospect as a full-time Care Partner in the Community Team for an expected period of 3 months. This role works between Carrington House & visiting clients in the community, with hours Monday - Friday 8am - 4pm. This is a great opportunity for care staff to try their hand at Case Management and supporting clients living independently at home. At the end of the secondment, you may have the opportunity to remain in the role, or you have the right to return to your current position.
The Opportunity
As the Care Partner you will guide older people through their care journey and Support at Home funding and services with compassion & clear communication.
- Delivering high standard case management & service coordination that is informative and responsive to individual goals and choices.
- Collaborating with participants and representatives to deliver an individual goal plan that supports assessed needs, goals and package resources.
- Working with the Clinical Care Partner to coordinate clinical care delivery within the quality framework.
- Ensuring that service delivery is within participant allocated service budget & managing unspent fund balances for the participant.
- Monitoring service outcomes, ensuring continuous improvement and client satisfaction, and that key KPIs are met
- Supporting all participants with navigating My Aged Care and aged care services as required.
- Participating in an on-call roster (remotely) to ensure timely service provision.
Requirements
- Qualification in Aged Care, Community Services, or a related field; (or equivalent work experience).
- Experience in care services working with diverse clients & communicating with families.
- Understanding of the Strengthened Standards, Aged Care Act and related regulatory frameworks.
- Excellent communication and interpersonal skills, with a compassionate and client-focused approach.
- Driver’s License & willingness to travel to clients in community
- First Aid Certificate (or willing to obtain)
- Knowledge of My Aged Care, Support At Home (SaH), Home Care Packages (HCP) and/or Commonwealth Home Support (CHSP) Programs
