Our Hobart office is a multidisciplinary team of OTs, Speechies, Physios, EPs and BSPs, supporting clients across the NDIS and Support at Home programs.
What You’ll Do
Based at our Hobart office, you will provide OT services to Support at Home clients in the community. Support at Home enables in-home aged care clients to remain protected, independent and engaged within their own homes.
Your responsibilities will include:
Conducting functional assessments and documenting client needs in detailed reports.
Communicating identified needs with clients, families, and service providers, and coordinating appropriate referrals to relevant health specialists or specialists.
Prescribing suitable equipment and recommending home modifications to support client independence and safety.
