Job Description

We are seeking a compassionate and motivated Community Services Worker to join our team. In this role, you will support individuals and families by providing person-centred services that promote independence, wellbeing, and community participation.

Key Responsibilities:

Provide practical and emotional support to clients.

Develop and implement individual support plans.
n
Maintain accurate case notes and client records.
n
Collaborate with families, community organisations, and other specialists.
n
Advocate for clients and connect them with appropriate services.
n
Ensure all work is completed in accordance with organisational policies and relevant legislation.

Essential Requirements:

Certificate IV or Diploma in Community Services (or equivalent).
n
Excellent communication and interpersonal skills.
n
Ability to work independently and as part of a team.
n
Current driver's licence and reliable transport.
n
National Police Check and Working with Children Check (or willingness to obtain).
n
First Aid and CPR certification (desirable).
n
