Are you a passionate and dedicated leader with experience supporting individuals with disabilities and guiding teams of support workers? Sirv is looking for an exceptional Team Leader to join our team in HORSHAM, Victoria, overseeing our Supported Independent Living (SIL) home and ensuring high-quality care for our participants.

About The Role

n

n
Oversee the daily operations of our Supported Independent Living homes.
n
Develop and implement support plans and risk assessments tailored to participants' individual needs and goals.
n
Ensure restrictive practices reporting is completed accurately and submitted to relevant authorities (e.g., NDIS, DHHS) within required timeframes.
n
Work collaboratively with families, teams, and external stakeholders to achieve positive outcomes for participants with highly complex support needs.
n
Guide and support Support Workers, ensuring they provide care in line with support plans.
n
Lead by example—providing hands‐on support to participants and promoting a strong, person‐centered care approach.
n

About You

n

n
Experience in supporting individuals with disabilities,



particularly those with complex needs.
n
A Certificate IV in Aged Care, Disability, or Community Services (or equivalent), along with a current First Aid/CPR certificate.
n
Proven leadership skills in managing and supervising teams in SIL homes.
n
Training & experience in Manual Handling.
n
Experience working with individuals with Intellectual Disability, Epilepsy, Schizophrenia (highly regarded).
n
Experience in managing rosters.
n
A valid NDIS Worker Screening Check.
n
Happy to work in an workplace with cats.
