As a Care Partner, you will coordinate services that support older people and people with disability to live safely and independently in their homes and communities. You will work closely with clients, families, support workers and other stakeholders to develop tailored care plans, manage service delivery, coordinate staff and ensure high-quality outcomes.
Key responsibilities
Conduct client assessments and develop person-centred care plans
Coordinate and support a team of Care Workers
Manage client services within funding and budget requirements
Complete regular client reviews and home visits
Monitor risks and implement appropriate support strategies
Maintain accurate documentation and client records
Assist with recruitment, onboarding and staff development
Ensure compliance with NDIS, Aged Care and organisational standards
Respond to client, family and staff enquiries professionally and promptly
About you
Certificate III in Individual Support, Community Services, Business or equivalent
Experience supporting older people and/or people with disability
Robust leadership, communication and organisational skills
Experience coordinating services and staff in a community care environment
Sound knowledge of NDIS and/or Aged Care standards
Excellent computer and technology skills
Queensland Driver's Licence and reliable vehicle
Current NDIS Worker Screening Check, Police Check and Blue Card (or ability to obtain)
