Key Responsibilities
About you:

We are looking for:
Minimum 1 year experience in community services or social support
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Diploma of Community Services (or equivalent)
A compassionate, reliable, and proactive attitude
Ability to build positive relationships with clients and stakeholders
A commitment to promoting inclusion and independence
Skill & Experience
Tasks & responsibilities:
Advocate for clients' rights, access, and inclusion within the community
Provide person-centred support tailored to individual needs
Develop and implement individualised support plans
Support clients with community engagement and participation
Promote independence, wellbeing, and life skills
Refer clients to appropriate specialist services and agencies
Maintain accurate documentation and progress notes
Work collaboratively with team members and external providers
Be part of a supportive and values-driven team
