Metro North Health is seeking a physiotherapist to join the Residential Transition Care program (RTCP) in a temporary full time capacity.
The role focuses on delivering high-level physiotherapy services, case management, and collaborative care for older people transitioning from hospital to home or aged care facilities.
You will work within a multidisciplinary team, provide clinical advice, and mentor junior staff.
