Job Description
Hunter Current England Local Health District is seeking a Clinical Nurse Specialist to join the Virtual Heart Failure Support Program (VHFS).
This temporary part-time role delivers virtual post-discharge care for heart failure patients, empowering them to manage their condition at home and reduce readmissions.
You'll perform telephone and telehealth consultations, educate patients, coordinate care across acute, primary and community services, and support evidence-based heart failure management
