NDIS Support Coordinator (Permanent Part-Time or Full-Time)

We are seeking an experienced and passionate NDIS Support Coordinator to join our team on a permanent part-time or full-time basis.

About the Role:

n

n
Assist participants to understand and implement their NDIS plans.
n
Connect participants with appropriate supports and community services.
n
Build participants' capacity to achieve their goals and increase independence.
n
Coordinate services and advocate for participants when required.
n
Maintain accurate case notes and documentation in line with NDIS Practice Standards.
n
Develop and maintain strong relationships with participants, families, service providers, and key stakeholders.
n

To be successful in this role, you must have:

n

n




Minimum 2 years' experience working as an NDIS Support Coordinator.
n
Relevant TAFE or tertiary qualification in Community Services, Social Work, Disability, Psychology, or a related field.
n
Current Working With -Children Check (WWCC).
n
Current National Police Check.
n
Current NDIS Worker Screening Check.
n
Excellent communication, organisation, and problem-solving skills.
n
Solid knowledge of the NDIS, its legislation, and the local service network.
n
Current Australian Driver's Licence and reliable vehicle
n
