Job Description

DPV Health is seeking an experienced Support at Home Care Partner to work with our service teams. You will guide participants, carers and representatives in developing versatile, personalised, culturally appropriate care plans and connect them with local services and resources.

You will conduct participant visits, promote person-centred care, manage budgets and support quality standards across multiple sites. National Police checks and WWCC are required.
