Job Description

Alfred Health is seeking an experienced Social Worker to serve as the NDIS Social Work Navigator within the Trauma Social Work service. The role spans across the Trauma stream and supports participants receiving care from the NDIS.

Solid assessment, case planning, and discharge planning skills are essential. You will work in a multidisciplinary team, contribute to quality improvement and supervise staff and students while addressing psychosocial barriers for patients and their families within
