The role supports inpatient and outpatient rehabilitation across Orthopedics, Cardiac, Neurology, Pulmonary and geriatric populations, working within a friendly, multidisciplinary team.
You will liaise with community services and families for discharge planning and aged care placements, with guidance from senior social workers.
