Care Hub Australia is seeking compassionate, reliable, and motivated Support Workers to join our growing team. In this rewarding role, you'll provide person-centred support to NDIS participants, empowering them to live independently, achieve their goals, and actively participate in their community.

You'll work closely with participants in their homes and the community, delivering supports that promote dignity, choice, and independence while building genuine, trusting relationships.

Key Responsibilities

Provide in-home care and day-to-day support tailored to each participant's individual needs.
n
Assist participants with personal care, including hygiene, showering, dressing, grooming, and toileting where required.

Support participants with daily living activities to maximise their independence.
n




Assist with grocery shopping, errands, and accessing essential community services.
n
Perform light domestic assistance, including meal preparation, laundry, vacuuming, mopping, dishwashing, and general household cleaning.
n
Support participants to attend appointments and participate in community and social activities.
n
Encourage skill development and independence in line with individual NDIS goals.
n
Maintain accurate progress notes and communicate effectively with the broader Care Hub Australia team.
n
Deliver supports in accordance with NDIS Practice Standards and Care Hub Australia's policies and procedures.
