Job Description

HammondCare is seeking Dementia Support Coaches to deliver in office and remote education and support across dementia care settings. The role focuses on relationship-based support, mentoring care staff, and tailoring DREAM modules to meet service needs.

You will collaborate with staff and leaders to implement evidence-based practices and enable person-centred care for people living with dementia. Fixed term contract until June 2028, with hybrid work options from Chermside (QLD) or Malvern (VIC)
