We are working with a local community care organisation to appoint a dedicated and compassionate Care Partner to support older Australians living at home through Support at Home packages.
n
This role is ideal for experienced Community Enrolled/Registered Nurses seeking a rewarding transition into case management. You will build robust client relationships, lead recent client onboarding, coordinate personalised care plans, and work closely with care teams to ensure high-quality services are delivered.

Key Responsibilities

n
Develop and maintain trusting relationships with clients and families.
n
Conduct onboarding, care reviews, and ongoing coordination using a wellness and reablement approach.
n
Create tailored care plans and service agreements.
n
Identify opportunities to expand services through client needs and community connections.
n
Promote services to support growth and enhance outcomes.
n
Work collaboratively with Caregivers and provide constructive feedback.
n

About You
n
n
Current AHPRA registration
n
Experience in aged care, case management, or Home Care Packages.
n
Comprahensive knowledge of Support at Home framework.
n
Experience in sales, growth, or business development is desirable.
n
Solid interpersonal and communication skills.
n
Confidence using Microsoft Office and CRM systems.
n
Valid Australian Driver's Licence and willingness to travel across Brisbane North & Moreton Bay.
n
Passion for empowering older Australians.
