Role Description

We are seeking an accredited Home Medicines Review (HMR) Pharmacist to join our growing network on a flexible, contract basis, with the ability to conduct in-person consultations within your preferred local area.

You will receive GP-referred patients through the Chemist2U platform and be responsible for:

Liaising with patients for in-person consultations
n
Preparing structured, high-quality reports using provided templates
n
Collaborating with GPs and healthcare teams to optimise patient outcomes
n
Providing practical, patient-centred medication advice to improve safety and adherence
n

Qualifications & Requirements

n

n
Current registration with AHPRA as a pharmacist
n
HMR accreditation (or eligibility to obtain)
n
Robust clinical knowledge of pharmacology and medication management
n
Experience in conducting patient consultations and education
n
Excellent communication and report-writing skills
n
Comfortable using digital tools for telehealth and documentation
n
Ability to work collaboratively with GPs and healthcare providers
n
Experience in community or remote pharmacy practice (desirable)
