Randstad Healthcare is currently seeking a Support at Home Case Manager / Care Partner for an urgent 3-month contract with Heywood Rural Health in South West Victoria.

What You'll Be Doing:
n
Community Case Management: Conduct home reviews, risk assessments,



and individualised care plans under the **** Support at Home program.
n
Workforce Support: Guide and supervise local Health Care Workers (HCWs) for secure care delivery.
n
Multi-Disciplinary Collaboration: Partner with local GPs, Allied Health, and participant families.
n
Budget & Compliance: Monitor funding allocations and maintain documentation
(TRAKCare/RiskMan).
n
What You'll Need to Apply:
n
Qualifications: Degree/Diploma in Case Management, Social Work, Psychology, Nursing, or Allied Health.
n
Experience: Background in community aged care, CHSP, or NDIS support coordination.
n
Mandatory Checks: Valid VIC Driver's Licence, National Police Check / NDIS Clearance, and current Flu Vaccination.
n
Working Rights: Full Australian working rights (sponsorship unavailable).
