Family Spirit in NSW is seeking an entry level Case Manager for the Permanency Team. You will develop and implement case plans for children and young people, providing ongoing therapeutic intervention and case management in collaboration with stakeholders.
You will engage with children, families and carers, undertake assessments, and support transitions within the permanency program, while maintaining records and complying with legal and policy requirements.
