Our services have a strong focus on prevention and early intervention and enable access to protected accommodation options, Holistic case management, community outreach programs and programs that teach vital independent living skills.
Our centres are safe and supportive environments creating an ideal environment for growth, where participants feel believed in, valued, listened to, and respected.About The RoleWe are seeking a dedicated Senior Case Manager to provide advanced case management support, specialist practice guidance, and quality assurance expertise.
You will be working with young people facing complex challenges, including homelessness, family violence, mental health concerns, alcohol and other drug (AOD) issues, and family breakdown.
Through intensive, therapeutic, and evidence-informed interventions, you will empower young people to build resilience, achieve positive outcomes, and create pathways toward stability, wellbeing, and independence.Reporting to the Outer South Program Manager, this is a permanent full-time position,
based on-site in Leongatha, VIC.ConditionsCompensation is in accordance with the SCHADS Award Level 6.How You Will Make An ImpactProvide specialist case management and consultation for young people with a range of support needsDemonstrate a strong commitment to working within a trauma and psychologically informed environment (PIE)Ensure support provided to young people is strengths-based and aligned with the Salvation Army Youth Stream's Journey to Independence Model of CareWork as a member of the Outer South leadership team to ensure the Leongatha youth services team practice is compliant with expectations around risk, safety and governanceSupport staff through expert guidance on complex cases, risk management, and critical incidentsCollaborate with key stakeholders to strengthen service integration and outcomes for vulnerable young people across the Gippsland regionWhat You Will BringTertiary qualification in Social Work, Welfare, Community Development or related fieldA minimum of three years' experience in a social service environment preferably in the youth sectorWorking knowledge of the challenges, risk,
