Our model of care provides holistic health and wellbeing services to support our Community across their lifespan. About the role
As Care Coordinator CCSS - MATSICHS, your role is responsible for specialist community and primary health nursing services that focuses on the prevention, early detection and management of health problems for Aboriginal and Torres Strait Islander people.
The role will work collaboratively with patients and designated general practices to facilitate a person with a chronic condition to manage their health in a way that will result in an optimal health outcome. As a member of a multi-disciplinary team ensure effective screening, service delivery and administration practices are delivered in accordance with the IUIH Model of Care.
The role will entail service delivery from designated primary clinic locations and outreach services, under limited supervision, as required.
Your day to day duties may include:
- Provide effective first point of medical engagement including, triage assessments, taking of medical history/detail,
assignment of patient, completion of preliminary procedures.
- Provide routine screening services to patients including blood pressures, blood sugar levels, temperatures, urine testing (routine and pregnancy tests), weight, length and head circumference (in children under three).
- Treatment of wounds and other clinical procedures as directed by the doctor.
- Attend to home visits with doctor if required.
About you
- Current C Class Drivers Licence (QLD).
- AHPRA Registration.
- Bachelor of Nursing Qualification.
- Current CPR Certificate.
