About the Role

We are seeking an experienced and compassionate Clinical Care Partner - Registered Nurse to join our Support at Home team in Cleveland.

As the Clinical Care Partner RN, you will play a vital role in supporting participants with diverse and complex health needs by providing advanced clinical nursing assessment, care coordination, clinical consultancy, nursing interventions, care planning, delegation and clinical leadership. You will act as a key clinical link between participants, their families and the health and community services that support them.

In this role, you will combine advanced clinical nursing expertise, care coordination, delegation, clinical leadership and relationship-based practice to support participants in achieving their goals while promoting wellness, independence and re-ablement. Acting as a clinical resource and subject matter expert, you will support evidence-based practice, contribute to clinical governance, lead clinical decision-making and assist in building workforce capability across the Support at Home program.

You will provide clinical leadership, supervision and oversight to Care Partners and nursing staff, ensuring delegated clinical activities are delivered safely, effectively and in accordance with professional nursing standards, organizational policies and the Strengthened Aged Care Quality Standards.

Key Responsibilities

- Provide advanced clinical nursing assessment, care coordination and clinical consultancy for participants with complex care needs.
- Identify, monitor and respond to clinical deterioration and emerging clinical risks.
- Undertake comprehensive health assessments to identify clinical needs, risks, strengths and opportunities to support independence, wellbeing and re-ablement.
- Conduct clinical nursing assessments and reviews for participants with varying levels of complexity.
- Develop, implement, monitor and review clinical components of individualized care plans.
- Review and evaluate participant outcomes to ensure clinical interventions remain effective, appropriate and aligned with changing needs.
- Deliver community-based nursing interventions within scope of practice and approved Support at Home services.
- Support wound care, medication-related needs, continence management, skin integrity and other clinical interventions.
- Support medication management processes, including medication reconciliation,



participant education and collaboration with pharmacists and prescribers.
- Monitor participants with chronic and complex health conditions and implement strategies that promote self-management and reduce avoidable hospital admissions.
- Participate in hospital discharge planning, care transitions and multidisciplinary case reviews to ensure continuity of care.
- Coordinate referrals and communication with GPs, specialists, hospitals, allied health professionals and other service providers.
- Support participants and families to navigate health, aged care and community service systems.
- Act as a clinical resource and subject matter expert, leading clinical decision-making for participants experiencing complex, escalating or changing health conditions.
- Provide clinical leadership, guidance and support to Care Partners and nursing staff, promoting evidence-based practice and high-quality participant outcomes.
- Delegate nursing activities and care interventions in accordance with professional standards, organisational policies and scope of practice requirements.
- Provide clinical oversight, monitoring and supervision of the nursing workforce to ensure safe, effective and person-centred care delivery.
- Review nursing practice, clinical documentation and care delivery to ensure compliance with professional standards, clinical governance requirements and organisational policies.
- Support, develop and monitor the competency of nursing staff through coaching, mentoring, performance feedback and clinical support.
- Facilitate clinical handovers and communication processes to support continuity of care and effective team coordination.
- Provide education, coaching and health promotion activities to participants, carers and families to support informed decision-making and self-management.
- Support advance care planning and end-of-life care coordination where appropriate.
- Promote participant choice, dignity, cultural identity, independence, wellness and reablement.
- Escalate safeguarding concerns and contribute to the management of participants experiencing vulnerability, neglect or abuse.
- Support infection prevention and control activities, including participant and staff education where required.
- Maintain accurate, timely and contemporaneous clinical documentation and records.
- Provide oversight of clinical services delivered by Associated Providers through brokerage arrangements to ensure quality, safety and continuity of care.
- Contribute to clinical governance, quality improvement, accreditation activities, clinical audits, incident investigations, clinical risk management and corrective action initiatives.




- Support compliance with organisational policies, legislative requirements and the Strengthened Aged Care Quality Standards.
- Contribute to service innovation and workforce capability initiatives that enhance participant outcomes and service quality.

About You

- You are a compassionate and confident Registered Nurse who combines sound clinical judgement with a person-centred approach and demonstrated leadership experience.
- Bachelor of Nursing.
- Current AHPRA Registration as a Registered Nurse.
- Demonstrated leadership experience.
- Experience in aged care, community nursing, primary care or a related clinical environment.
- Demonstrated experience assessing and managing participants with complex or changing health needs.
- Demonstrated ability to delegate, supervise and support nursing staff in accordance with professional and legislative requirements.
- Experience leading, mentoring and developing clinical staff within a community, primary care or aged care workplace.
- Strong clinical assessment, risk management and clinical decision-making skills.
- Understanding of clinical governance principles and the Strengthened Aged Care Quality Standards.
- Excellent communication, interpersonal and relationship-building skills.
- Ability to work collaboratively within a multidisciplinary team.
- Strong documentation, care planning and organisational skills.
- Ability to work autonomously while recognising when escalation or specialist clinical advice is required.
- Commitment to participant choice, dignity, cultural identity, independence and reablement.
- Current Driver Licence and willingness to travel within the community.
- Current First Aid and CPR certification (or willingness to obtain).
- Ability to meet all probity requirements, including an NDIS Worker Screening Check.
- Experience in restorative care and reablement.
- Knowledge of advance care planning.
- Experience using electronic care and clinical documentation systems.
- Experience working with culturally diverse communities.
- Postgraduate qualifications in aged care, community nursing, chronic disease management, clinical leadership or a related discipline.
