The Clinical Care Coordinator is responsible and accountable for supporting the Registered Nurses and Care staff in the clinical management of the facility, the delivery of clinical quality care, maintenance of standards and promotion of a positive work setting.

We are looking for 1full time Clinical Care Coordinator for our facility at Cootamundra (Monday to Friday).

Responsibilities include

- Monitor the care plans of residents
- Act as an adviser in response to the complex nursing and care needs of all residents
- Practice continuous improvement activities to maintain a high standard of client care
- Monitor the implementation of care plans and regularly review and update as appropriateManage each resident’s condition and progress, and ensure that documentation is regularly evaluated in accordance to SCC guidelines and accurately reflects ongoing care
- Liaise as required with Doctors, Paramedics, hospitals, health care professionals and other specialist service providers
