About the Role
Our client is a well established not-for-profit supporting people with disability to live independently across Perth. They are seeking a Registered Nurse to join as a Clinical Care Coordinator, coordinating high quality, person-centred services for clients.
This role provides day to day supervision of Support Workers, contributes to workforce planning and roster management, and ensures services are delivered safely and consistently. Acting as a central point of coordination, the Clinical Care Coordinator works closely with clients, families, support workers, practitioners and external stakeholders to translate client goals and practitioner recommendations into practical, measurable support outcomes.
Key Responsibilities
- Provide clinical leadership and advice to the management and rostering team on client clinical needs
- Formally review client care plans to ensure compliance with standards of care
- Ensure individualised, quality service delivery for clients receiving personal care and support
- Manage relationships with internal and external stakeholders to support service delivery outcomes
- Lead a team of personal support workers to ensure high quality service delivery
- Monitor client funding packages against agreed budgets and address issues as needed
- Partner and collaborate with clients and stakeholders to deliver appropriate, sustainable services
- Regular local travel by car required, with occasional travel to remote and regional areas by air
About You
- Registered Nurse with current AHPRA registration
- Experience in community services or the disability sector highly regarded
- Ability to work within a quality management and compliance framework
- Strong organisational skills, able to manage multiple priorities while delivering excellent client outcomes
- Proven experience leading, supervising, mentoring and developing staff
- Advanced communication and stakeholder relationship skills
- Ability to rapidly acquire knowledge and work within a quality framework
- Current Australian driver's licence and reliable vehicle
- Current NDIS Worker Screening Check and Working with Children Check, or willingness to obtain
