Seed Disability Services (NDIS Registered Provider) is seeking for a friendly, reliable, and proactive support worker to join our team and support a young participant.

Your role will include:

- Assisting with personal care and day-to-day support needs
- Supporting attendance at GP and other health appointments
- Helping with community outings, activities, and events based on the participant’s interests
- Providing simple daily wellbeing check-ins and flagging any concerns to the team




- Assisting with light household tasks to support daily living
- Delivering supports 1:1 and in small group settings
- Completing NDIS-compliant shift notes after each shift

Requirements: NDIS Worker Screening Check First Aid & CPR Driver’s licence & reliable vehicle (preferred)

Police Check Person-centred, reliable, and qualified
