- Advocate for participants and support them to exercise choice, control and access to appropriate services.
- Manage a caseload of NDIS participants while delivering high-quality Support Coordination.
- Maintain accurate documentation, reports and case notes in line with NDIS Practice Standards and organisational requirements.
- Assist participants to navigate NDIS, prepare for plan reviews and maximise plan outcomes.
- Coordinate, review and oversee service delivery to ensure supports are meeting the participants requirements and goals.
- Build participant’s capacity and confidence.
- Provide expert advise and consultation to NDIS participants and their families around navigating NDIS.
- Build robust relationships with participants, families, providers and key stakeholders.
- Identify and manage participant risks or concerns.
