Everyday Independence is seeking a Youth Lead Practitioner to support adolescents and young adults with disability, guiding them through education, personal and social goals. You’ll work across homes, schools and community settings within a supportive, multidisciplinary team.
You will lead and mentor a youth-oriented caseload, coordinate NDIS therapy packages, and collaborate with families to build sustainable routines while progressing through meaningful outcomes.
