Cabrini Health in Melbourne's southeast seeks a Project Coordinator - Cognitive Care to lead a hospital-wide program focused on delirium, dementia and cognitive impairment outcomes. You will translate evidence into practice, embed sustainable clinical systems, and collaborate with Nursing, Allied Health and Education teams.

The role is fixed-term, part-time (0.6FTE) for 12 months, with a robust emphasis on governance, education and change management to improve patient care and safety.
