Prime Care Partners is seeking a Care Partner to join our Support at Home team, coordinating services for clients across the Perth Metro area.

This is a full-time role, 8.30am - 4.30pm, Monday to Friday, with one evening per week on-call.

This role has the opportunity to work both in the community and from home minimum one day per week.

The Role:

This role supports clients under the Support at Home package, providing care coordination to older adults.

The Care Partner will be responsible for delivering care management activities that enable older people to live safely and independently in their home. The Care Partner will collaborate with clients, carers/families, and multidisciplinary care teams to ensure rights-based, person-centred care that is tailored, responsive and inclusive.

Care Partners will provide ongoing planning, monitoring, service coordination and education to older adults, and family.

Other responsibilities include:

- Collaborating with clients, representatives, families, support workers and health professionals,

- Reviewing and following up client clinical issues, hazards and incidents,

- Implementing and monitoring compliance requirements under the Aged Care Quality Standards, and

- Building professional relationships with clients, representatives and families.

Role Essentials:





This role requires the successful candidate to hold a current driver’s licence, and a clear National Police Clearance.

Ideally, the successful candidate will have knowledge of aged care, the Aged Care Quality Standards, Support at Home, or experience in care coordination.

The role will suit someone who is passionate about providing quality care and support to clients. It requires someone who is organised, manages their time well, and has a good understanding of computers and technology. Interpersonal skills and the ability to build and maintain strong professional relationships are a must in this role, as well as the ability to work with diverse clients to achieve the goals and outcomes they choose.
