You will deliver recovery-focused care to older adults in the community, provide assessment, treatment, follow-up and work within a multidisciplinary team to support wellbeing and prevent deterioration.

You will hold AHPRA/NMBA registration, nursing qualifications and be committed to evidence-based practice, service integration
