The successful candidate will be responsible for conducting comprehensive assessments of older people, identifying their care needs, and supporting appropriate recommendations and referrals. This role requires robust clinical assessment skills, excellent communication, and a genuine commitment to supporting older Australians.
Key Responsibilities
Conduct comprehensive aged care assessments and clinical assessments.
Assess clients' physical, psychological, functional and social needs.
Develop appropriate care recommendations and referrals.
Complete accurate clinical documentation and reports.
Liaise with clients, families, GPs, allied health professionals and other service providers.
Participate in RAV-related assessment activities as required.
Maintain skilled standards, confidentiality and compliance with relevant legislation and guidelines.
Pay: $35.00 – $45.00 per hour
