Key Responsibilities
Provide person-centred support in accordance with each participant's individual needs and support plan.
Assist participants with daily living activities, personal care, meal preparation, household tasks, and community participation where required.
Support participants to develop and maintain independence and achieve their personal goals.
Build positive and respectful relationships with participants while maintaining professional boundaries.
Provide companionship and engage participants in meaningful activities.
Assist with transport and community access where required.
Follow participant support plans, medication procedures, and workplace policies.
Complete accurate progress notes and other required documentation after each shift.
Identify and report any incidents, changes in participant wellbeing, or concerns to the appropriate team members.
Maintain a safe, clean, and supportive environment for participants.

About You

Requirements
Certificate III in Individual Support, Disability, or an equivalent qualification
NDIS Worker Screening Check.
Current First Aid and CPR certificates.
Medication Competency Certificate.
Valid Driver's licence and access to a reliable vehicle.
Relevant experience in disability support or community services is desirable.
Right to work in Australia.
